Behavioral Health Provider

Mountain Community Health Partnership is seeking a full time Behavioral Health Provider. Primarily day shift hours. Mon – Fri, with occasional weekend. The Behavioral Health Provider (BHP) is responsible for providing integrated behavioral health services to Mountain Community Health Partnership (MCHP) patients; especially under-served, uninsured, under-insured, high-risk and vulnerable patient populations residing in the service area of MCHP regardless of their ability to pay for services. These duties include serving as a direct provider of behavioral health services providing quality, compassionate, and culturally sensitive care for patients of MCHP according to evidence-based best practices for integrated behavioral health care. The BHP supports the care teams in coordination of patient care by provider or case manager referral and shared visits.

Requires

Master of Social Work (Other educational backgrounds may be considered depending on clinic needs). Minimum two years supervised employment, preferably with experience in primary care. Current North Carolina Licensed Clinical Social Worker and Licensed Clinical Addiction Specialist. Current BLS certification. Accuracy in data collection using EMR, and keyboarding skills required. Behavioral Health Nurse Care Navigator

Mountain Community Health Partnership is seeking a full time Behavioral Health Nurse Care Navigator to work collaboratively with our team in order to provide effective and timely care to patients with health problems and opioid use disorders. Primarily day shift hours. Mon – Fri, with occasional weekend.

Responsibilities:

• Conduct initial assessments and intake as part multi-disciplinarian team, obtaining social, medical, mental health and substance use and medication history.
• Provide direct patient care to patients with opioid and substance use disorders, screening and telephone triage, performing intakes, supervising initial buprenorphine induction, and providing education on treatment options for substance use disorders.
• Monitor the health status, treatment progress and medication adherence of the Office Based Opioid Treatment (OBOT) patient panel.
• Review intake assessment with the team and create an appropriate plan of care for the patient.
• Work with the multi-disciplinary team to address needs and progress through the revising of treatment plan, as needed. Co-facilitate group medical visits with Behavioral Health Team to increase healthy behaviors, reduce stigma of opioid use disorders and support wellness and recovery.
• Communicate and collaborate with care team providers, including Primary Care Provider (PCP), Behavioral Health Provider (BHP), Program Coordinator, Peer Support Specialist and other team members as deemed appropriate.
• Revise and update patient treatment plans with Collaborative Care Team, as needed, to address patient needs and issues.


Patient Education and Communication

• Educate and assess patients looking to access behavioral health and treatment for Opioid Use Disorders using medications.
• Utilize screening and monitoring tools to identify treatment needs. Monitoring of patients in the induction, stabilization and maintenance phases of treatment.
• On-going education and support of patients in all phases of substance use disorder treatment including: nursing visits schedule and unscheduled, urine toxicology screens, routine labs, medication teaching, monitoring, pill counts, observed dosing and medication refills.
• Follow state and federal guidelines in providing care to patients with opioid use disorders.
• Conduct ongoing primary care of patients referred for OBOT and behavioral health program.

Quality Improvement

• Attend weekly Team meetings.
• Ability to interact and communicate, both orally and written, with a variety of people.
• Work with team members, including Primary Care Providers to obtain medication orders, prescriptions, refills and communication as needed.
• Collaborate and coordinate care with BH team members around mental health needs, psychiatric emergencies and crisis-related clinical concerns.
• Regularly update patient substance use disorder patient registries needed.
• Commit to a focus on continuous improvement of workflow processes and procedures.

Summary of Qualifications

Required:
• Bachelor’s Degree in Nursing
• 3 years’ experience with patients with mental health and substance use disorders required.
• Current, active, unrestricted NC Registered Nurse licensure
• BLS Certification

Preferred:

RN with Certified Addictions RN credentials or the desire to obtain certification.
• Bilingual in English and Spanish is a plus.
