Medical and Preventative Care Affordability Program MCHP’s Medical & Preventative Care Affordability Program is a sliding fee scale program for people who are uninsured or underinsured and have incomes at or below 200% of the federal poverty level. The sliding fee scale is based on your family size and income. This allows you and your family to pay a reduced cost for covered medical services at MCHP locations.

To apply for our sliding fee scale, you will need to complete a Sliding Fee Application and provide proof of income and family size. Eligibility is extended to all patients, regardless of other medical coverage, with the exception of NC Medicaid. (Patients with Medicaid coverage for Part B Medicare premiums may be screened for sliding fee).

Patients may be insured and still qualify for a sliding fee. Oftentimes we have patients with limited financial means who have Medicare or private insurance, but these individuals may face barriers to paying deductibles, co-insurance, etc.

To apply for our Affordability Program, please call or come by one of our locations in Bakersville, Burnsville, Celo, or Spruce Pine. Interpretation services are available.
