About Us
As one of the largest networks of community health centers in our area, MCHP offers a wide variety of health care services to meet the needs of our community, with four medical locations and a dental center. We have a team of dedicated and compassionate providers and staff members who focus on caring for the whole patient, regardless of their ability to pay. We bring care and wellness to more than 10,000 patients annually and have been committed to community based rural health since 1947.Â

FQHC
As a Federally Qualified Health Center, we are a resource for wellness, prevention, and quality care. We provide primary care, prenatal care, women’s health, pediatric care, behavioral health, and more at all of our offices. For a more in-depth explanation of our services, click here. Family Practice
Mountain Community Health Partnership providers are dedicated to caring for the whole person. We have providers that are board-certified in family medicine, a specialty that recognizes the person as part of a family and community and takes care of each patient according to their stated needs. We provide complete health care for all ages and genders, emphasizing prevention, addressing urgent needs, and helping each patient manage any long-term conditions that develop.
Patient-Centered Medical Home
Our health centers have received the highest level of recognition as a Patient-Centered Medical Home (PCMH) from the National Committee on Quality Assurance (NCQA). The PCMH is a model of patient care delivery that is team-based and led by the patient’s choice of health care provider who provides continuous and coordinated care to maximize health outcomes. The patient and family are members of the team and are an integral part of decision-making.

Advocacy Center of Excellence
An Advocacy Center of Excellence (ACE) is a Community Health Center that creates a culture of advocacy to ensure that policymakers at all levels of government commit to investing in the affordable, equitable, and innovative care that health centers provide. ACE levels recognize consistent engagement and demonstrate an ongoing commitment to making advocacy an organizational priority. ACEs actively partner with the National Association of Community Health Centers (NACHC) on federal policy issues, as well as their state Primary Care Association (PCA) on key local-level policy issues impacting health centers and their patients.
Just Economics' Living Wage Certification Program identifies and promotes local employers that pay all employees a Living Wage. Their program is currently the largest of its kind in the country. Living wage-certified employers are making an invaluable contribution to their communities by paying employees a wage that is sufficient to provide for their needs. We hope this recognition will inspire other organizations to follow suit and ensure their employees receive fair and livable wages.
